Health Adjustment Passport

Use the passport to describe the support that helps you move into, stay in or return to work.

The Health Adjustment Passport is a DWP document for people with a disability or health condition. It can help identify support and adjustments, support an Access to Work application, and make conversations with employers easier.

What it can help with

Put the work barrier and support need in one place.

The passport is designed to be updated as work, support or job circumstances change.

01

Workplace adjustments

Record support and changes that may help remove barriers at work.

02

Access to Work

Use the passport to support an application for extra in-work support where the scheme applies.

03

Changing roles

Carry useful support information into a new job, manager or work situation.

Before you complete it

Describe the work need, not just the health condition.

A stronger starting point is: what task is difficult, what barrier appears, what has helped, and what support may be needed next?

  1. 01Work goalWhat job, role or return are you preparing for?
  2. 02BarrierWhat becomes difficult in practice?
  3. 03TaskWhich part of the job is affected?
  4. 04SupportWhat change, equipment or help may reduce the barrier?
  5. 05Who helps?Employer adjustment, Access to Work or another route?
  6. 06ReviewWhen should the support be updated?

Free preparation worksheet

Prepare the useful parts before opening the official form.

Choose your work stage, main barrier and likely support need. The worksheet gives you prompts to think through before completing the DWP passport.
